Augustus and Hazel decide to share their favorite novels before they bring Hazel home. Augustus gives Hazel the book The Price of Dawn and Hazel recommends An Imperial Fliction.

Hazel discusses the beauty of An Implacable Affliction. It's a book about Anna, a little girl suffering from cancer. It's the only book she's ever read which reflects her experiences. She describes how the book abruptly comes to an end in the middle of the sentence and leaves the reader with no closureas to the fates of the characters. She speculates on Peter van Houten's mystery author of the novel, and she believes was a fugitive from Amsterdam within a short time after the publication. The author hasn't been seen since then.

Augustus unexpectedly informs Hazel that he came across Van Houten's secretary Lidewij and was successfully able to establish an email communication with her. Hazel is able to share the letter of Van Houten with the other, and they come up with an agenda to inquire about Van Houten questions. This is an attempt to help clarify the novel's confusing conclusion. Hazel is concerned about Anna's fate. Hazel believes that Anna's mother will survive her daughter's passing and that her parents will be fine after Hazel's demise. Van Houten eventually replies that he's unable to respond to Hazel's inquiries in person. He invites her to visit him when she's in Amsterdam.

Augustus is invited by Hazel for a lunch soon after. He turns out that he has planned a Dutch-themed picnic. He informs him that Van Houten an organization that grants wishes to children with cancer, has offered to give him his wish. He touches her face and she is thrilled. She is able to see that she loves him, however, she is sure she will harm him if she dies. She likens herself to a grenade.

Hazel experiences an extreme episodeof fluid accumulation within her lungs. She is taken to the ICU. After a few days, Hazel is finally released and discovers that Augustus is not leaving the hospital's waiting area. Hazel receives a second note from van Houten. The second letter is more personal and obscure. Hazel is more determined to travel to Amsterdam than ever after reading the letter. However, there's a problem: her parents and team of doctors aren't convinced she's strong enough to travel. It seems hopeless up until Maria, Dr. Maria, one of her most experienced doctors, is able to convince Hazel that she should travel in order living her dream.

They leave Van Houten's in total disappointment, and are accompanied by Lidewij who is horrified by Van Houten's behavior. Augustus and Hazel exchange a passionate kiss at the end of their trip to cheers of spectators. They then return to their hotel and are reunited in the very first moment. Augustus admits to Hazel that he was scanned by the body scanner when being within the ICU. The scan revealed that his cancer was back and spreading across. Hazel finds out that Augustus will be the one to grenade that is thrown when she returns to Indianapolis. The health worsensand it is evident that he has lost his charisma and confidence. Even though he's more vulnerable and afraid yet he's beautiful to Hazel. The moment she stops calling him Augustus and instead refers to him as Gus as his parents. Hazel realizes that she has a love for him. Augustus's condition deteriorates quickly. Augustus organizes a prefuneral with a prefuneral service. Isaac and Hazel offer tributes. Hazel takes a line from Van Huyten about larger and smaller infinities. She shares with Augustus how much she cherishes her and how she would never wish to miss their short time together.

Augustus dies eight days later. Hazel is shocked to meet Van Houten in her funeral. Van Houten said how Gus and he had a long-standing correspondence, and that Augustus demanded that Van Houten rectify the mistake he made on his excursion to Amsterdam. Van Houten also stated it was appropriate that Hazel was required to attend his funeral. Van Houten discusses Anna's fate abstractly, however Hazel does not care. Hazel is told by Isaac within a couple of days that Augustus had written a letter to her. As Hazel attempts to find the letters, Van Houten appears again. He is drunk and reveals that Anna was the name of his daughter. At the age of eight old, she passed away from cancer. A Imperial Abilities was his literary attempt to reconcile himself with her death. Van Houten is advised by Hazel to stay sober and begin writing a new book.

Hazel is later informed she was Augustus given the pages directly Van Houten because he wanted VanHouten to write a thoughtful eulogy about Hazel. Lidewij makes Van Houten read the pages and then sends them to Hazel. Hazel is able to finish the novel by reading the words of Augustus. He said that while it's normal to be injured in the world, we do have the choice of choosing to allow others to do so. And he was satisfied with his choice. He hopes that Hazel will be happy with her decision. Hazel says that she likes the way the novel ends.
